COVID QUESTIONS
Concerns about COVID?
I have fully vaccinated.
IF YOU DESIRE:
1. I can wear a mask (and will bring some with me).
2. I can wear disposable gloves (and will bring some with me)
3. I can wear disposable shoe coverings (and will bring some with me).
4. I will wear a hospital coat, to remain clean and protected.
5. You can provide and use your own art materials (if you chose).
6. I will have art materials with me that will be cleaned and sanitized between use.
Health Questions:
Prior to making your appointment and confirming your appointment, we will go over a few questions:
1. Have you or any of your household members had a fever in the past 48 hrs?
2. Have you or any of your household members felt unwell or ill (flu-like symptoms)?
3. Have you or any of your household members been vaccinated (not required)?
4. Have you recently been exposed to someone diagnosed with COVID in the last two weeks?
